NSIB Investigates Fly Bird Aircraft Emergency After Dual Engine Failure

 

The Nigerian Safety Investigation Bureau (NSIB) has commenced an investigation into a severe aviation incident involving a Fly Bird HS 125 aircraft, registered as 5NKAL, that experienced dual engine failure mid-flight. The incident occurred on December 6, 2024, during a flight from Abuja’s Nnamdi Azikiwe International Airport (DNAA) to Kotoka International Airport in Accra (DGAA).

According to a statement by Mrs. Bimbo Olawumi Oladeji, Director of Public Affairs and Family Assistance at the NSIB, the aircraft carried four people onboard, including three crew members and one passenger. The plane returned safely to Abuja after a critical in-flight emergency.

While climbing through flight level 240 en route to flight level 280, the crew reported an issue with Engine No. 2. At approximately 17:54 UTC, when the aircraft was 25 nautical miles southwest of VOR station VONUK, the crew requested permission to return to Abuja. Clearance was granted, and the aircraft began descending to flight level 190.

However, the situation worsened when Engine No. 2 failed entirely at flight level 230. As the aircraft approached Nnamdi Azikiwe International Airport at around 5,000 feet, Engine No. 1 also failed, prompting the crew to declare a Mayday. Despite these challenges, the skilled crew managed to execute a safe landing at 18:16 UTC. Remarkably, no injuries were reported.

The NSIB has launched a comprehensive inquiry to determine the factors behind the dual engine failure. The agency's Go Team is collecting evidence, interviewing involved parties, and analyzing the aircraft's flight data and maintenance logs.

“This investigation is critical to preventing future incidents and ensuring aviation safety,” the bureau said in a statement. “Further updates will be provided as new information becomes available.”

This incident underscores the importance of rigorous maintenance and contingency planning in aviation. While the crew's professionalism and training averted a disaster, the loss of both engines in flight is a rare and concerning event that demands thorough scrutiny.

The NSIB’s findings are expected to contribute valuable insights to bolster safety protocols and enhance the reliability of aviation operations.
